Abstract Aims
The NIHR funded SAFER-YCL project developed an end-user adapted version of the SAFER-YMH care bundle for use in CAMHS Crisis and Liaison services with the potential to enhance transition experiences and improve discharge efficiency. Our aim was to explore how this care bundle could be digitalized and made accessible to young people, carers and healthcare professionals through existing NHS digital infrastructure.

Methods
Focus groups were held with NHS IT and clinical applications professionals, young people and parents/carers to explore their perspectives about jointly held forms. Analysis followed a Normalisation Process Theory framework.

Results
Participants determined the feasibility of a digital, patient and carer accessible version of the SAFER-YCL care bundle, hosted through existing NHS digital infrastructure and mapped the necessary steps required to build and implement this digital care bundle. Young people and parents/carers identified what mental health care documents they would like to be in an accessible form.

Conclusion
This study determined the feasibility of a digital, patient and carer accessible version of the SAFER-YCL care bundle, hosted through existing NHS digital infrastructure. In doing so, we have identified the mental healthcare documents that young people and their parents feel should be jointly held and how these documents should be accessible. If successfully implemented, we would expect to improve care experience and improve service efficiency.
