| Abstract |
Aims: In severe cases, outpatient psychotherapy and pharmacotherapy may not be sufficient. Multiprofessional home based treatment is an alternative to inpatient treatments, allowing the maximum involvement of the whole family and facilitating the transfer of therapeutic benefits into the everyday life of patients. In this workshop we will show possiblities and difficulties, advantages and disadvantages of home based treatment compared to classical treatment approaches and will discuss when multiprofessional home based treatments might be indicated. Methods: A monocentric pilot study (BASe) has been conducted from 2020-2023. A German multicentre trial (HomeBASE) has been set up and apart from clinical effects, the cost effectiveness of need-adjusted multiprofessional home-based treatment will be examined. Results and Discussion: In the pilot study, home based treatment was conducted after inpatient treatment. We found a stronger reduction of overall psychopathology (child behavior checklist total score) during home based treatment compared to treatment as usual. However, shorter inpatient treatment durations and a shift of symptom reductions from the inpatient treatment interval to the outpatient treatment interval could account for this effect. Home based treatments also show limitations, when a patient group is needed to activate patients or when hospital resources are needed in acute phases of eating disorders. We discuss how multiprofessional home based treatment can reduce long term costs in severe child and adolescent psychiatric disorders. |
