| Abstract |
Acompanya’m Lleida started activity on December 2023. It is a residential therapeutic and educational center for children under the age of 18 with a severe mental disorder of high clinical, family and social complexity, who have a torpid evolution and does not allow treatment in community resources or other hospitalization services. Is the second center Acompanya’m in Spain, both integrated into the public health network. The center is structured in five apartments of living units, 20 beds on total. The unit’s care model from the clinical perspective organizes the whole social, educational, training and family environment of the child, priorities the needs of the children in their own natural cohabiting environment promoting the recovery of the person’s life project. The aim is to offer therapeutic, social and educational care in an intensive, personalized and interdisciplinary way (health and social professionals). The unit aims to establish a therapeutic environment with a high capacity for containment in which self-and/or heteroaggression is minimal. Intervention based on a positive discipline and affectivity model, achieving the minimum use of possible restrictive measures. The positive discipline model focuses on respectful parenting based on emotional connection. It focuses on fostering skills that promote autonomy, responsibility, and respect, fostering social skills and emotions while setting clear and consistent boundaries. Professionals from all disciplines carried out training in positive discipline. 19 children and adolescents have been attended with an average age of 14, 4 discharges have been carried out. 47% were female and 71% had two or more mental health diagnoses, the most prevalent being autism, conduct disorder, major depressive disorder, post-traumatic stress disorder, borderline personality traits. 53% came from families at risk of claudication. Restrictive measures have been applied to all patients. During the first year of functioning of the center, recorded in the medical record results were: total restrictive actions=184; under stimuli room (observation)=99; Seclusion=85; Mechanical restraint=0. Reasons for using restrictive measures were: psychomotor agitation (n=65), suicidal ideation (n=20), somatic prevention (n=1), risk of absconding (n=10), reduction of stimuli (requested by the patient) (n=88). The care model, the training of professionals and teamwork suggest positive results in the use of restrictive measures. |
